In the UK i've only ever heard it in the first two senses. Never heard it as 3 or 4. However my shorter OED does have an entry mentioning 4 though as a rarity. Even in senses 1 & 2 it's not common, a bit flowery. most common use is for the most types of lorry, fixed or artics (semis?), where it'...
